Treatment of MCL with irutinib monotherapy

This study was based on a meta-analysis of 370 MCL patients and a 3.5-year follow-up. The patients were mainly treated with irutinib. As part of three open-end studies, the follow-up was cross conducted in three studies, including additional exposure treatment. This study has specific requirements on the time and method of using irutinib and the exposure time.

After 41 months of follow-up, the complete remission rate was 26.5%, and the median survival time was 26.7 months. The 2-year overall survival rate was 53% (95% CI, 0.47-0.58), the 3-year overall survival rate was 45% (95% CI, 0.39-0.50), and the 5-year overall survival rate was 37% (95% CI, 0.25-0.49). After the first year, the number of new events decreased. The most common teaes above grade 3 included neutropenia (17.0%), thrombocytopenia (12.2%), pneumonia (11.9%), anemia (9.5%), atrial fibrillation (5.9%) and hypertension (5.1%) - most of them occurred within one year of irutinib treatment. There were 229 cases (61.9%) of emergent serious adverse events (SAEs) after treatment, and the initial SAEs decreased with time.

Comparison of single drug therapy with chemoimmunotherapy in the treatment of CLL

There are still some limitations in this study, mainly due to the lack of data on patient level available in chemoimmunotherapy, as well as the difference between study design and patient eligibility criteria.

Among the patients aged 61-74 years, long-term PFS and irutinib monotherapy have better advantages than chemoimmunotherapy, except for some patients with specific complications, especially for patients with advanced diseases, such as patients with bulky lymph nodes. In elderly or less healthy patients, the effect of single drug of irutinib is better than that of chemical immunotherapy, and the adverse reaction rate of no less than grade 3 is lower than that of chemical immunotherapy.

Despite the limitations of this study, the authors suggest that their cross-over study suggests that erlutinib may potentially eliminate chemotherapy in patients with newly diagnosed CLL.

Quality of life in the treatment of CLL with irutinib monotherapy

The observational data of this study were obtained from the phase III clinical study of elderly patients with newly diagnosed CLL. Over a period of 18.4 months, compared with the treatment with nitrogen mustard, irutinib alone reduced disease progression or mortality by 84% in the patient population.

In patients aged 65 and older, they were randomized to receive 420 mg of irutinib once a day until the disease progressed, or to use nitrogen mustard phenylbutyrate for 12 months. The reported results of various patients were evaluated by global health status, etc. In the follow-up of 269 patients with progressive bone marrow failure (38%), lymphadenectasis (37%), splenomegaly (30%), fatigue (27%), night sweats (25%), etc., it was found that compared with the treatment of nitrogen mustard, irutinib significantly prolonged PFS, and reduced disease deterioration or mortality by 87%.

The focus of this kind of special research is to make greater and lasting improvements in pros, and many of its research projects are still improving over time. Based on a certain analysis model, after an average follow-up of 18.4 months, there was no disease deterioration or the average time of grade 3-4 toxicity was longer.
